Online Forms Instructions


Thank you for using Rank One Sport. You will use the student athlete’s school ID number to login. You may choose to create a new account or continue as a guest. If you have multiple children participating, you may use the same account to complete the online forms for each child. Upon approval of your completed online forms, you will receive email verification.

REQUIREMENTS FOR PHYSICALS FOR THIS YEAR

All incoming 7th, 9th and 11th graders are required to have a new physical for the 2026-2027 school year. If you have answered questions 1-6 on the medical history as a “Yes”, your child may need a new physical. If your child suffered a concussion, has a history of cardiac problems or has had a surgery within the last year, your child will need a new physical for the 2026-2027 school year.

The Medical History is online and all yes answers must be explained.

The Physical document must be turned in and it is kept on file with the athletic trainer.

To complete the Online Forms, move your cursor to “Electronic Participation Forms.” A dropdown menu will appear. All three sections must be completed by the student athlete AND parent/guardian. Within each section, all lines must be completed as well. If a line is not applicable, please click “N/A” where it is available, otherwise enter “N/A” into the data field.

To sign the forms electronically, click the signature field and sign your name with the mouse while holding, left click. If you make a mistake, you may click the refresh icon and try again.

These forms may be completed on mobile devices as well.

Upon completion, please, do not turn in hard copies. You may print each document for your own records, or screen shot if you so choose.
